War of 1812- Declaration of a Widow for Pension
Concerning Jacob Rooksberry

County of Floyd
     On this 19th day of November, A.D. eighteen hundred and seventy nine personally appeared before me, Clerk C of the Circuit Court, a Court of Record in and for the County and State aforesaid, Mary or Polly Mayfield, aged ninety two years, a rsident of Edwardsville, County of Floyd, State of Indiana, who, being duly sworn according to the law, declares that she is the widow of Levi Mayfield, who served the full period of sixty days in the (1) military service of the United States in the War of 1812, and who was the identical Levi Mayfield who (2) volunteered in Captain Kelly's Company, Col. Callaways Regiment, 8th Ky Military Brigade, General Harrison's Cov'd Division at Middletown KY, on the 28th day of August 1813, and was honorably discharged at Maysville KY, on the 1st day of December 1813; that (3) he was in the battle of the Thawes~
That her said husband was sometimes called Isaac Mayfield as well as Levi and she does not know whether he was enrolled under the name of Levi or Isaac~ neither claiment or her husband were ever ?? married~and she has not again married since his death
that she was married under the name of Polly Rooksberry to said Levi Mayfield, on the 14th day of July, A.D. 1814, by Rev. Benjamin Allen, at Boston, Jefferson Co., Ky, there being no legal barrier to such marraige, that her said husband did at Floyd County Indiana, on the 18th day of January 1839, and that she has not remarried since his death; that at no time during the late rebellion against the authority of the United States  did she or her husband adhere to the cause of the enemies of the Government, giving them aid or comfort, or exercised the functions of any office whatever under any authority, or pretended authority, in hostility to the Untited States; that she will support the Constitution of the Untited States; that she is not in reciept of a pension under any previous act; that she makes this declaration for the purpose of being placed on the pension rolls of the United States, inder the provisions of the act approved February 14, 1871, and hereby constitutes and appoints, with full power of substitution and revocation, M.M. Carr of Louisville Ky her true and lawful attorney to protect her claim and obtain the pension certificate thta may be issued ?? her post office is at Edwardsville, County f Floyd, State of Indiana; that her domicile or place of abode is Edwardsville Floyd County Indiana-
